Despite all of these conversations, there is still so much confusion around hormones and menopause. Every woman’s journey through peri/menopause is unique and the symptoms she may or may not experience can vary over time. Not every woman experiences hot flashes and night sweats — some women experience sleep disruption, frequent waking,  and low energy. Others experience the hormonal change as intense anxiety and uncertainty and loss of a sense of self. Some women feel the changes more physically with debilitating brain fog and/or achy joint pains. Others have disruptive sweats all day and night. Some women don’t note any obvious symptoms while others check off all of the boxes. There is no right or wrong way to go through menopause. 

The purpose of hormone replacement therapy is not to ‘fix’ or ‘reverse’ menopause — it is simply to support a woman’s quality of life through this transitional time.

By utilizing small doses of the same hormones that are dropping during perimenopause we can blunt the severity of the symptoms that are associated with those hormonal changes. 

Perimenopause tends to get all of the attention because the symptoms associated with rapid hormone changes can be so significant. We often hear women say ‘I can’t wait for my period to be over and this to be done with’. The good news is that the symptoms can often ease up… but the bad news is that other more gradual physical changes come from the permanent loss of hormones. The post-menopausal hormone changes include slow changes in metabolic health markers like blood sugar and cholesterol levels, the ability to gain lean muscle mass, the loss of bone density, changes in skin elasticity/plumpness, and changes in the vaginal tissue.

Hormone replacement therapy (HRT) is a safe and effective strategy to support women struggling with symptoms of hormonal changes in perimenopause and menopause. Menopausal symptoms can occur for as long as 10 years (10 years!!!) before the last menstrual period and are associated with significant morbidity and negative impacts on quality of life. Perimenopausal and menopausal symptoms will affect every single body system from head to toe. Symptoms such as brain fog, heart palpitations, fatigue, low mood, anxiety, joint pain, painful intercourse, vaginal dryness, low libido,  headaches, weight gain and insomnia are also prevalent in combination with bothersome hot flashes. This wide array of symptoms can impact a woman’s ability to function at work, at home, and in life and can impact relationships with their partner, family members and kids.

Who can prescribe hormone replacement therapy (HRT)?

We understand that HRT is a safe and effective option for so many women, but it may be difficult to access a healthcare provider who is experienced in treating and supporting women with hormone replacement therapy.  All the Naturopathic doctors at the NutriChem clinic can prescribe hormone replacement therapy to their patients as they have completed the extra qualifications necessary under the standard of practice for prescribing by the College of Naturopaths of Ontario (CONO). Our team of naturopathic doctors and clinical pharmacists take a holistic approach to supporting women during this transitional period which includes lifestyle, nutritional and supplemental support as well as hormone replacement therapy.

The NutriChem clinic team utilizes bloodwork to check the response to hormone replacement therapy and assess baseline health status. Our lead women’s health pharmacist, Grace Meehan is also a member of NAMS — the North American Menopause Society.

What are bioidentical hormones?

Bioidentical Hormone Replacement Therapy (BHRT) is used to treat hormonal deficiencies or imbalances in both females and males but is most commonly used to treat symptoms of perimenopause and menopause. BHRT uses plant estrogens that are identical to our human sex hormones, which offers a natural alternative to animal-derived or synthetic hormones. Although BHRT can be given by prescription, the dose is standardized, leaving patients in a “one-size-fits-all” scenario.
